With over 27 validated care pathways and 5 million patient-days of remote management, the model is ready for replication — regionally, nationally, and beyond.

Providers can deploy validated care pathways or build off them, defining patient-specific thresholds and alerts. This adaptability enables the delivery of personalized care pathways that can simultaneously cover multiple conditions, preventing siloed programs that increase the burden for providers and patients.
