Eligibility check*
Do you have a Dutch health insurance? Do you meet the criteria to join the program? Complete the online sign-up form to do a quick check.
*If your GP already referred you, you can skip steps 1 and 2.
Referral from doctor or GP practice nurse
To participate, you need a referral letter from your GP with your latest weight and waist circumference. Your doctor or GP practice nurse can find us via their referral system. If they cannot find us, the referral letter can be submitted by mail to gli@ancora.health.
For reimbursement, it is important that you have a recently measured (less than one month old) weight and waist circumference. These measurements must be taken by your referring doctor or a nurse.
Welcome email
Learn more about the digital nature of the program and receive your login details for your personal Ancora Health account.
Informed consent and questionnaire
Using your login details in the welcome e-mail, you can sign in to your personalAncora Health account. Here you will be asked to give us your consent to proceed with the program and then be able to fill out your intake questionnaire and schedule your intake call.

Schedule your online intake call where you will learn more about the program and make sure SLIMMER powered by Ancora is right for you.
First six months
Weekly: group coaching video calls and online chat
An enjoyable part of the program, group coaching sessions guide you on topics such as habit formation, healthy food choices, and getting active. Together with other participants, get advice from expert lifestyle coaches and inspiration from each other.
Your commitment:
- Join at least 22 of the 26 online group sessions during the first 6 months.
- Share your progress and successes.
- Be helpful to your group mates.

Monthly: 1-on-1 sessions with your personal lifestyle coach
You will have regular check-ins with your personal lifestyle coach. In total, there are five hours of one-on-one lifestyle coaching, including 9 video calls throughout the program (4 calls in the first 6 months). During the intensive phase, you will also have chat support for the first 6 months.

- Set goals and action plans together.
- Be open and honest with your coach.
- Share your obstacles, difficulties or achievements.
- Prepare your coach calls by telling your coach what you need.

Every day: lifestyle change
Life(style) is an adventure! Each day, you’ll take small but meaningful steps toward a healthier you, supported by our coaches and fellow participants. With the right tools, motivation, and guidance, we’ll help you stay on track and create lasting change.
Your commitment:
- Make the most of the program by reading the content, answering questions, completing assignments, and taking part in challenges.
- Keep track of your weight, waist circumference, nutritional intake, physical activity and more in the Ancora Health app.
- Consider joining physical activities happening in your local area.
- Regularly reflect on your progress by filling our questionnaires, mainly after 6 months and at the end of the program.

Months 7-24
Group coaching: video calls and chat
In our program, we believe lasting lifestyle change happens together, not alone. Support from those around you is key to success. That’s why we encourage connection and shared experiences. Many of our groups still meet regularly, benefiting from ongoing motivation, accountability, and progress.
Individual coaching: video calls
Check in once every four months with your coach to make sure you are achieving the results you want in a sustainable way.
Evaluation
At the end of the 2 years, you are invited to an evaluation call to review your experience with us and discuss your next steps.
Reimbursed by your health insurance
Age 18 years or older
BMI You have a BMI of 25 or higher and an increased waist circumference. (For women this is 88 cm or more. For men this is 102 cm or more.) Or you have a BMI of 25 or higher and have a health condition that is related to obesity (sleep apnea, osteoarthritis, cardiovascular disease, type 2 diabetes).
